I just solved it - I went to look at my own marriage and saw an option down
the bottom of the Marriage information window.
Married names
0 - None, Don't generate married names
1 - Replace wife's surname with husband's surname as an additional entry in
Index view

I had that on 0 - I switched it to 1 and I am now in the index under both
names

Then I went to Options > Customise > Data Entry - Rule for generating
default married names - and changed that to 1 as well
I'd set it to 0 when I first got the program as I mis-understood what it
would mean.

Thanks everyone for your help - your comments helped me look for what might
need changing to see the result needed.
