You can look for Joe Blogg's Birth Certificate in a folder containing
1000 files, or you can make a couple of extra clicks to get to the
Bloggs/Birth Certificates subfolder and choose from 20 files.  Makes
sense to me.

I don't actually split all the Source documents by family.  I have them
split into folders according to what they are (eg Baptism Images) and
subfolders (eg Newspapers/Title) and then name the file by
Surname-Forename-RIN, usually with a leading identifier of the file
type, such as BAP, or MC.  Only exception is Censuses which is one
folder per country per year and I name the images with the Census reference.
