I could use some help in choosing the best format for a medical history report.
My sister is heading to Mayo for help and needs a complete family medical history spanning at least 3 generations. We have a very large family. I need to create a report or chart that is easy for her and her doctors to read that includes the medical information in notes. I have made a separate family file that includes just the individuals needed in the report/chart, but I can't seem to find a format that works cleanly and completely with notes. Any suggestions would be greatly appreciated!
