When my family needed something similar, I tried several
approaches.  First, I tried a simple, straight-forward,
"Descendant narrative" type report which I edited to exclude
the unnecessary and include the necessary.  It only took me
a couple hours to realize this was far too time-consuming to
be useful.

Then, I tried the Medical History Chart from the US Govt
site; it too failed to satisfy.

[if you google "family health history" .gov you get a number
of hits for useful forms that aren't necessarily from a .gov
site.]

Finally, I simply did a GED of the relevant portions of my
main data-base into a new database, changed the NAME to CoD
(with Dx codes where available), color-coded for heart,
lungs, circulation, other; deleted all notes, and printed a
single-page pedigree chart and associated FGS.  I could've
stopped with the pedigree since that's all he read. :)
