I've just spotted one more situation when data loss is happening. To reproduce:
- open any outline - hit Ctrl-I to add a node - select the headline so you can edit it - change the headline, then press Ctrl-I to add a new node Important: press Ctrl-I when the headline is still selected and can be modified.
