I wrote some code for my ripper, cued, that extracts the Q sub channel from the raw 96 bytes of subchannel pack data. If you grab cued, the code that unpacks the Q sub channel is in the files pwsc.h and pwsc.c (which also use qsc.h and qsc.c.) I only unpacked Q because it was all I cared about at the time, but the code should give you some idea of how to pack it. The link to cued is https://gna.org/projects/cued/.
