Patch includes a little cleanup/reorg of adjoints_ex2 to test out adding
separate QoI object. Note that the internal API for the QoI functors in
FEMSystem had to be updated to provide a non-const diff_qoi pointer. OK for
trunk after a day or two on the list (assuming no one has any other ideas)?
