I didn't notice specific slowliness by opening queries or forms but it could be just suggestive. Just noticed that the report seems quite weird (but there were a lot of error messages in console). Could you give details about this slowliness (how many seconds for which step) ? Before to do this, could you uninstall all the LO versions (apt-get remove --purge), remove (or backup) your LO profile (found in ~/.config/libreoffice or ~/.libreoffice, check both of them).
