We are running Kernel 2.4.7, Kernel-patchlevel 2, dasdfmt patchlevel 4.
I have the latest Kernel (patchlevel 4) running as well with the latest
s390tools.

SuSE. I'm at home at the moment but Bernd have you got your ears on ???

Yes, there is at least a related changelog that I've found that describes
a crash after formatting a device.
If you would try a kernel with all fixes until the last drop, I'll send
it to you in a separate mail.
