Adam,

He's booting from the HMC CD drive.  The .ins file is used by the HMC to
locate the other files when IPLing, as well as what storage addresses to
load them into.

Maciek, where did you get the debian.ins file?  Did you make it up yourself?
If so, you might be better off creating a real tape, since I don't know what
values should go into that file.  For that matter, what _is_ in the
debian.ins file?

I don't know what debian.ins is doing there unless it's your TDF file.
The other three are in the right order.  Make sure that your TDF file
specifies the three files in the right order.  I also found once that I
had to pad parmfile out to 1K with nulls to make the emulated tape work
right, but that was on a Hercules system and I don't know if real iron
requires that.
