Did you format the volume with DSF CPFMT? OS/390 is looking for the
VTOC in physical cyl 0, which should have been put there by the DSF CPFMT or
INIT. OS/390 won't see the label and vtoc on cyl 1 unless it is
running under the same VM and the disk is linked to the OS/390
userid.
