The simplest way possible: my /dev/dasda is a 125 cylinder minidisk, which I
mount as /boot (Sorry, overlooked this critical piece of information
earlier.)

So, starting at 391:

391     125 cyl      /boot
392     10016 cyl   LVM vg_system)
               1gb             lv_swap
               2.5gb          lv_var          /var
               500mb        lv_tmp          /tmp
               remaining    lv_root         /
392     10016 cyl   LVM (vg_local)
               2gb             lv_home     /home
               remaining    lv_opt         /opt
