The easiest way to create a qeth device is to use the qeth_configure
command
 /sbin/qeth_configure [options] <read chan> <write chan> <control chan>
<online>
In your case you can try: qeth_configure 0.0.0c00 0.0.0c01 0.0.0c02 1

This will create the ccwgroup and bring it online, but it also creates the
udev configuration file

I'm running a DDR-copy of RH6 on a second-level z/VM 5.3 system.  RH6
certainly runs as a second-level guest.  But on boot third-level I see:
