Hi Johan, thanks for your offer to help! Fortunately the root cause is already identified as a bug in zipl (package s390-tools). If it triggers depends a lot on sizes/offsets - due to that it was there a long time undetected - theoretically you could have run into it before or even with other distributions by bad luck as well. So IBM already identified the issue (see Stefans mail on the thread a while ago) and in the meantime provided a fix - thanks to them!
This fix is already integrated in the Ubuntu development release, but yet in Xenial. Please see https://bugs.launchpad.net/bugs/1592990 for a recent status and links on how the fixes go into more Ubuntu releases.
