Hi Mark,  I had tried that echo command but the system would not allow me to 
change the file.  I ended up rebooting before seeing this message, that cleared 
whatever was preventing me from configuring it on r/w and allow me to format 
etc.

Using the dmesg command may provide some more info. If not, you can put udev 
into debug mode to see if you can get more information that way.

It may also be worthwhile to execute
echo 0 > /sys/bus/ccw/devices/0.0.012c/readonly
and see what happens.
