Nope, both the cord from the CPU fan and the power supply fan are
hooked up; fans appear to be blowing (and outwards, too)

possible, but seems unlikely -- this has happened with both sides and
the top cover off.

possible. i have a slot 1 celeron, and a heatsink/fan combo that has a
little metal chip that holds it on. Should the heatsink go on the side
of the CPU that's relatively flat (what I would call the back), or
aganist the side with the raised-up chip ("the front"). 

I had it on the "back".

When I just pulled all this stuff out, I did check again, and the CPU
is stamped "400/66 Costa Rica", and I was using the predefined 400/60
settings, so this:

  Rogier> - the CPU is getting WAY too much core voltage.

also seems unlikely.
