I think I can tell you what is happening, with stddisclaimer.h included.

In /etc/inittab, you have told the process to respawn at termination.
But instead of user, something else is terminating the process as soon
as it respawns. Actually, due to some misconfiguration, either the
process
receives a valid 'kill' or is abnormally terminated.
But since termination itself respawns the process, as mentioned
in inittab, it respawns. And you get the message. Basically,
it may be something relating to your device settings... I had the
same error when configuring the machine for a really old
Motorola UDS modem [9600 bps]
The problem was solved when the machine correctly knew what to expect
from the modem and vice-versa...
