I worked around this problem, could not fix it since I don't know the 
cause.
I first manually start pppd:
#/usr/bin/pppd
<lots of junk characters>
#kppp &
#
This works for me, maybe it will work for you. As I cannot find 
consistent conditions in which to reproduce the bug, I cannot send in 
a bug report either :(
