I don't known what exact procedure these ISP's use, but it is quite simple
that they provide these CD's containing reserved login ID and a password to
access their registration server (most probably a software) and same server
keeps tack of the time one has consumed.
Regular CD creation process could be enough no special numbering is needed,
however the registration cards may contain valid serial numbers for creating
new accounts. The user may be asked enter this as a temporary login ID or
password.
