Yes.  This week in fact.  I just got an IDE-CDROM to replace
my old mcd drive.  When I used the old Boot-Disk, (I was doing
a re-install), it gave me the error.

I forgot that the old boot-disk that I had mad was for installing
from an mcd drive, not a regular IDE-CDROM.

Bottom line:
Make sure the boot image you used to create your install-boot disk
matches your hardware!

Also, if you are using a non-standard location for hardware, 
give the parameters at the prompt.  mcd's are usually located at 0x300,
irq 10.  Mine is at 0x340, irq 10, so I said mcd=0x340,10.
