I use a cheap no-name v34 internal ISA modem, so maybe this will help.
The factory default is CTS hardware flow control. Probably that is OK
for windows, but it causes timing problems with any reasonable OS.
There is a command I can give it to make it use full RTS/CTS hardware
flow control, then it works fairly well. For my modem it is \Q3, but
you should read your modem manual carefully, yours may very well need a
different command.
Also, for some cheap modems, mine included, if you mIx caSe in a command
line, results ArE uNSpecifieD! Mine just seizes up and stops responding
for a time, but I have seen worse behavior, including setting the speed
to 300 BAUD!? Your ISP's modem will almost certainly hang up if it
hears osmething like that.
And it's not very good at negotiating a lower speed if say a v.32bis
answers at the ISP. When the ISP is busiest, I find it easiest to tell
it to select V.32bis auto mode. I have a script that sets the init
string based on time of day.
You should probably give chat the -v option and check your log to see if
you can get a clue what it's choking on. Your ISP will also hang up on
you if it doesn't like your login name or password. Does it hang up
before or after your modem shuts off the speaker?
Also, after it fails, use minicom or so to ask the modem about its
configuration. AT&V is _fairly_ standard for that, but check your
manual.
I find it best to drive the serial port as fast as it will go, and let
the UART regulate the effective speed with interrupts, so I use spd_vhi
and set the app to 38400, so the port runs at 115200.
Line noise matters, and the higher the speed, the more it matters.
Fluorescent lights and dimmer switches can generate quite an electronic
racket when they're in use, possibly enough to make a carrier
unrecognizable.

It should be different. It takes less time, and more melodic hissing
and screeching to negotiate 14.4 than 28.8. I can hear the difference,
but I can't explain it any better than that.
