That sounds like you are using the wrong driver, i have one of these
cards but i need to use the 3c59x driver, to make sure we are talking
about the same card (mine is a pci device) here is the output of
proc/pci

I suggest you make an entry in /etc/conf.modules or if you are using
the latest version of modutils its now /etc/modules.conf
alias eth0 3c59x
Nothing further is needed.
