Chuck i cant answer your question but, i would turn off the PnP with the
windows/dos program for that card.

I am also presuming (at a good guess) the card is simalar to the
RealTek8019, 8019 is the chip, you will need to use the ne.o driver.

Providing that PnP is turned off you can edit /etc/modules.conf or
conf.modules whichever you have and enter;

alias eth0 ne
options ne io=0x220
modprobe ne

You can read more on this in the Ethernet-HOWTO somewhere in your /usr/doc
directorys.
On my slackware system para's 3.4 and 5.31.3.

If you dont have the dos software to reset the card, the HOWTO tells you
where you can obtain it and howto use it.
