Sounds like you accidentally changed a DIP switch setting on it when taking
it out or putting it in.

You need to go to Adaptec's sight and download the manual for your card and
set the switches approriately. Note that the default 154x (and I'm pretty
sure 152x) drivers only see cards at 330 and 334. My 1542 has dip swtich
settings like this:

Here's the quick version of the DIP switchs for the 1542CF I have in my
hands:
Switches, from left to right along edge of card, circuitry facing up:
Up is off, down is on
sw1: termination
sw2,3,4: I/O port
000=330H
100=334H
010=230H
110=234H
001=130H
101=134H
011=reserved
111=reserved
sw5: BIOS on/off
sw6,7,8: BIOS address
000=DC000H
100=D8000H
010=D4000H
110=D0000H
001=CC000H
101=C8000H
011=reserved
111=BIOS disabled
So my settings are: floppy off, I/O: 330, floppy: off, BIOS: DC000H, which
are kind of the "factory default."
