Did you use a raw device?  You must use a raw device with ftformat.

Like this:

[root@godzuki doug]# ftformat -f /dev/nrawqft0

This command works to format a TR3 cartridge on my machine.  If you
using another drive, you might want to check the man page or
ftformat -h for other commands to get it working with your hardware.

I'm at a loss if this doesn't work.  If you wish to install the newer
version of ftape, make sure you get one of the later unstable betas from the
development directory.  I'm using "ftape-4.x-1999_06_30.tar.gz" and it works
well with my machine.  The very latest tarball wouldn't compile on my
machine; I haven't had time to see why.  I'm not in much of a hurry to
change my working setup, either.

Beyond that, I haven't any good suggestions.  I believe you said you had the
tape drive working with your current hardware, so it must be something on
the software side...
