It looks like the device/driver "rescan" is dying somewhere.
My situation - if the device is plugged in when the driver is loaded, all is OK. If driver is loaded, then device is plugged in, the driver isn't associated. So USB works for devices that are plugged into the root port, with modules. Any external hubs aren't getting associated with the hub driver (so downstream devices are dead).
