Likely it's a device flaw, as it Jan's problem.  The device is choking on a
perfectly legal command.

In Jan's case, he can hack scsi_scan.c to avoid generating the offending
command.  This fix is in 2.5.x

In your case, it's difficult to be certain until we see logs, but it's very
likely to be an already-known and mostly-fixed-in-2.5 issue.
