so in this case the output gets written to the file 91CD2.iso and not to
stdout.
Not on my Mandrake system. That modifier just changes the file name of 
the output ISO. The stdout is where that file is placed. On my system, 
it outputs it into the /home directory of whoever the user was that 
executed the command (I searched for the file and found it there).

no, just specify the path:
of=/path/to/91CD2.iso
