I have one of these - very easy to set-up with a series of set-serial commands - 
KNOPPIX does it automatically with kudzu.  SCUTTLE http://scuttle.netfirms.com was 
selling these for $50 some time back - don't know if he has any left though.

Well, I emailed several vendors with the question:
"Do you have a real hardware PCI modem? By 'real' I mean one
with a controller that appears to the Operating System as a
serial port ready to accept AT commands."

I got a reply offering one of these:
