Make sure the work machine has a static IP on the work network. 

In the ADSL Modem / Router at work set up TCP port forwarding for ports
5800 and 5900 to the work machine running the VNC Server. 

Make sure that any firewalls allow the traffic through to the work
machine. 

Make sure you've set a default password for the VNC server on that
machine. 

>From there you should be able to use vncviewer under linux to connect to
the office IP and have the connection forwarded to the VNC server
machine.
