Hmmm, PDF creator works pretty well. It's acts as a printer on your
(Windows) desktop, combines files (not just PDFs) into a single PDF format,
and allows you to enter document metadata for the PDF for *free*.
Additionally, you can set up PDF Creator as a print server for both Linux
and Windows documents. PDFSam only appears to have part of that
functionality.
