I think I've worked this out. ./.INBOX must be a directory and must 
contain a file "maildirfolder", otherwise Courier (or replacement), won't 
know that ./.INBOX.Foo is a subfolder INBOX/Foo of INBOX. Correct?

Even wierder than I thought...
