I use a version of the Classification Notes form available at 
http://www.greece.k12.ny.us/instruction/ela/6-12/Tools/  Many other styles 
and tools there also.

Landmark's Citation Machine (http://citationmachine.net/) is a good match to 
the OSLIS citation maker. Both have their own strengths; I check them both to 
see which works best for the resource types your SS teacher wants to use.
