I am just redesigning mine as you sent this! What is your goal? Do you
want to use independent reading as a "workshop" to incorporate lessons
in your curriculum, or are you looking to have them read for enjoyment,
or both? 
I tend to want my 8th graders to read for enjoyment, but I want to keep
track of what they're reading, what they're "abandoning" and what they
like. 

My goal was to encourage them to read for fun, but also to keep them
accountable for what they were doing without requiring reports,
journals, or extensive projects at the end of each book. We read class
books together along with independent reading, so they're already doing
those things. I needed something easy to grade, but something that keeps
them on track, so I designed it myself -- it's based on the Reader's
Notebooks that I've seen published, but I made it to my own specs. I can
send it as a word attachment if you give me your email.

Hi,
As a rule, I simply lurk on this site, but I'm trying to re-incorporate
independent reading back into my language arts curriculum.  I'm looking
for
help in designing both an independent reading contract for
students/parents
for each trimester and reading log for them to keep and turn in during
the
trimester.  If any of you have and use one or both, I'd be interested in
hearing from you.
