Actually it doesn't. It's designed to use one as a feature but it's not
required.

They are - the ABI is managed by the projects concerned, and they are
actually pretty good about it within major versions.

That doesn't need ISO standardisation just people willing to do the no
commercial value work for the fun of it (like Debian).

You don't have to standardise obsolete things. They don't tend to change
anyway. CP/M has been perfectly ABI stable since about 1982.
