This seems to be more a LaTeX than a LyX matter. Check out some
LaTeX books on this.

I never did an index, but at least LyX_1.0pre2 has a menu entry
(Insert->Index Entry...) to do something like this. You may want to
update LyX from ftp://ftp.devel.lyx.org and again check some LaTeX
book about this.

No problem. The latest LyX version has more support for this, even
some documentation.
Briefly, you create a new and empty LyX file and via Insert->Include
File... You give "pointers" to the chapter files. You can even
specify to temporarily ignore some of these files. This speeds up
compiling.
