I don't understand what you want exactly.

The underscore _ in math mode can be created with the command

\_
(as described in chap. 16.4 of my documentation)

If you need some text or a word surrounded by quotes " in math mode, use mathematical text (shortcut M-m m). The problem is, that quotes in mathematical text are always ", the menu layout -> document -> language has no effect (possibly a bug).
To have low quotes anyway, insert two commas. To create high quotes insert two grave accents (line accent from upper left to lower right). This will give you high quotes with the form of a 66 (The rule is, that quotes in different heights have the form: 99 hello 66 )


(good fonts will recognise two commas as low quotes and two grave accents as quotes with the form 66)
