In Document > Settings > Math Options, uncheck "Use AMS Math package
automatically" and check "Use AMS Math package". Alternatively, you can
add \usepackage{amssymb} to the preamble. If LyX had noticed other
things in the document that required AMS Math, it would have loaded the
packages automatically, but apparently it did not.
