This is a strange one, I was totally in the same boat as you with my iPone 4 - it was unuseable without the workarounds you mentioned.
I still had people saying it wasn't happening to them and that it was just me! Now have a 4S and I can say it is a lot lot better. I can't say it never happens, it happened about an hour ago, but it seems that if you get it to your ear smoothly, it starts off on the earpiece and stays there, if it's funny at the start your best ending the call and calling them back. I'd say it's 99% better than before. Of course, if you are waiting untill August, we'll be only weeks away from the iPhone 5 and you might as well wait for that one! Happy new year.
