Hello Jim

I recommend you use AudioBook Builder from Splasm Software.  The application 
has been very well tested with accessibility and I myself have worked with the 
developers to make it so.  In fact, they have been kind enough to work with Mac 
Access and they are recommending us as a first stop for visually impaired 
users.  But that's not really the point.

There are free apps out there but I haven't found one which comes close to 
AudioBook Builder.  Sometimes it's better to do things properly and sometimes 
that involves paying.  But it isn't expensive and it's so useful.
