You can only purchase items from the iTunes store or App store that is 
associated with your account, and that's generally associated with the 
geographic location of your credit card.  Or, if you create an account just for 
the purposes of downloading free apps, the App Store used is the one associated 
with the geographic information of the full address you supplied in creating 
that account.  However, you can change the location of the store that is 
displayed in iTunes on your Mac.  I do this to check the availability or 
pricing information of apps in other stores when I'm posting from my Mac, and I 
want to find out whether an app is available to users in another country before 
I post.

On most pages in the HTML area of the iTunes Store or in the Mac App Store, the 
last element is a link image for "My Store".  For example, if I move to the 
last element in one of my iTunes Store pages with VO-End (that's 
VO-Fn-Shift-Right arrow on my laptop), I'll hear VO say "link image My Store: 
United States, Choose Store".   If you activate that link, either by pressing 
the up and down arrow keys simultaneously in Quick Nav mode, or with VO-Space, 
you'll bring up a page with a heading for "Choose your country or region".   
Navigate by headings (VO-Command-H, or just "h" if you have single key web page 
navigation enabled with Quick Nav under Lion in the "Commanders" section of 
VoiceOver Utility, Quick Nav tab), then navigate to the country you want for 
that region and activate either the link image or the link with VO-Space or by 
simultaneously pressing the up and down arrow keys in Quick Nav mode.  iTunes 
will reload the home page, but the entry will be for the new country you 
selected.  You can check this by using VO-End again, and  the announcement will 
read the new store.  However, you'll also be signed out of your iTunes account 
when you switch stores.  To check this, stop interacting with the HTML area and 
either VO-Left arrow or just Left arrow if you have Quick Nav on.  You'll find 
a "Sign into my iTunes account" button. If I've changed my store and attempt to 
purchase items in the other store, or if I activate this sign in button, after 
entering my login and password I'll get a dialog window with the message: "This 
Apple ID is only valid for purchases in the U.S. iTunes Store. You will be 
switched to that Store. Try your purchase again." Once I press return or the 
"OK" on the dialog window I get taken back to my original iTunes Store.  On the 
Mac App store there will also now be a "Sign In" link under the "Quick Links" 
heading of pages such as "Featured", "Top Charts", and "Categories" if you 
changed your store location.

If I'm checking on iOS apps or looking for music, I usually go directly to the 
"Search…" page of the iTunes Store from the iTunes menu bar's Store menu.  That 
keeps the "busy" messages down, since I'm not loading the image and link rich 
HTML content of the iTunes Store home page. Then if I want to change countries 
I can use the Web rotor (VO-U) and switch the rotor to "Images" with the arrow 
keys.  On the power search page the "My Store" link image is the only image 
that will show up in this list.  Laptop users might find that easier than 
VO-Fn-Shift-Right Arrow, which I press by sliding the middle three fingers of 
my left hand up slightly from the Fn, Control, and Option keys so my middle 
finger presses the Shift key, and then I press the Right arrow key with my 
right hand.
