Have you looked into local businesses such as typesetting shops (probably renamed to something else by now.) They have the capabilities to print from disc and bind the pages into books. Can't hurt to make a few calls if someone has the time. And if you already have the product from Apple, you might want to have a face-to-face with the shop to see if they can match or at the least come close on the cost.
