Firstly, you should be able to connect an external SCSI CD ROM drive
(connected via the external HDI30 connector), put a system CD into it and
boot from it by holding the C key down. This is always the best way to deal
with problems of this sort. If you dont have an external SCSI CD-ROM drive,
you should be able to get one from eBay pretty easily, or I might have one I
can send you for the cost of the postage (or where are you located - email
me offline). If you want to keep on using your PB145 its a very worthwhile
thing to have.

The internal drive in a PowerBook is a 2.5 model and I cannot recall how
you would connect it to a desktop Mac with a 3.5 SCSI interface. Easier to
fix it in situ using the approach I suggest above.

Indeed it might, if the HD would even show up. HD SC Setup doesn't
even want to recognise the drive at all. As for CD, I think that
would need me to AppleTalk the PB with my Centris 610 (which has a CD
drive), share the CD and hope that the minimum system that boots from
a floppy is prepared to see the CD on the network. This may be worth
trying too.

If it is a driver issue, due perhaps to corruption at the start of
the disk, do I then need to pull the disk and try it in another
machine? The cable seemed to have fewer pins than standard SCSI, but
I could be mistaken, as it's a while since I opened up the Centris.
