I don't know if there are any set rules for doing this. I do the following:
(1) I check with the old ISP to see if there's an auto-forwarding that can be done, even after I turn off the old account. (This is assuming you are turning off the old account.) If you're keeping the old account, then poke around the settings for an auto-forward option.
(2) Send all the people I regularly correspond with a notification that I'm moving.
(3) Subscribe under the new address to to any mailing lists I want to keep and, after the new address kicks in, unsubscribe under the old address.
(4) Remember to change the old address on any Web pages, Facebook, etc.
