My local Apple shop tried replacing various bits, but in the end they swapped the whole thing. It gradually got worse, worked OK for a few weeks, then the battery started complaining, but Cmd-Alt-R-P would help for a while. In the end it would only ever get as far as rEFIt if I left it off for at least 20 minutes, and the LED on the charger wouldn't light up at all. At that stage, neither I nor the local shop techie were able to get it to boot off the firmware update CD. I politely asked for a report from them when they got any results from the central Apple repair people, but I've not heard anything yet. For kicks, I left it running while I was away (11 days) and it would appear that the battery is now somewhat confused again: after a reboot the batteri is "present" and "charging", but rate, capacity and voltage are all zero. Someone mentioned that the applesmc module isn't quite up to par with what MacOSX does to the hardware - I guess that might be the reason.
