Yes, I restarted my iPhone first thing before I even tried to get the volume 
back using the speaker. Nothing doing. In fact, this is so bad, that if I 
didn't have a sighted person present, I have not a clue how I would get the 
volume back. The only thing I didn't try is to reset it with the home key and 
power key together. 

Although I think they probably know about this by now, I just wrote to 
Accessibility.
