Hi, If, without VO sound, you have difficulties determining whether VO is on or off and various other things, note the following:
* If you simply press the physical Unlock switch, then attempt to adjust the volume, it will not be changed. * You must get to the regular Home screen or to your Passcode screen in order for the volume up/down buttons to activate. * Make sure the Silent mode switch is not on Silent mode by sliding it towards the face of the iPhone. * If VO is on when the physical Unlock button is pressed and you here nothing, touch the centre of the screen, swipe right four or more times, then swipe left once, then double-tap. This will take you to the next screen. Whether you have Passcode enabled or not, you can now press the up volume and the volume changes should actually be applied. * if the above step doesn't work, then right after you press the physical Unlock switch, triple tap the Home button, which should automatically turn VO on, unless you've set it differently. You can then follow the steps mentioned above. As David mentioned, volumes for various outputs are controlled separately so that's why you likely could hear sound depending on whether headphones were plugged in or not. Later...
