Under Yosemite, Preview does not play very well with Voiceover as yet. Tim is right that you need to interact twice in order to read the usual way. The trouble is that often when opening a pdf document with Preview, even though you may have interacted twice, you might not be able to see the text nor navigate it with vo+arrow keys. I’ve reported it to Accessibility at Apple but please do so as well if you find your experience is similar. The work around it is to open and close a document and then open it again. Sometimes even this won’t work. So another way of extracting text is to open the document, pressing command+a to select all the text and pasting it into a new TextEdit document. Alternatively, you can use Nisus Writer to open a pdf file if you have either Nisus Writer Pro or Express.
