Well then, at least your system sound is working, that makes it easier. Do you 
have a Mute Speech command set up in any commanders? I've done that before - 
hit the hotkey and forgot I did so. Is speech volume all the way down? Have you 
hit cmd-f5 four or five times very quickly? Sometimes this can restart 
VoiceOver and get things working again, or so people say.
