Actually, it makes sense, in a way. The content shown inside the frame comes 
from a different source and is loaded into it, and because it comes from a 
different page, the JavaScript cookie isn't recognized using the setting and 
the fields don't show, but instead shows you the warning. This is evident from 
looking at the page source alone, even on both sides including the frame 
source, but if you go here:
http://clients.mindbodyonline.com/ws.asp?studioid=37742&stype=-2&subTab=schedule
with the setting set to only permit cookies from website you visit, it will 
show. I'm not sure what prompted such a design decision, and thankfully it is 
rare. At least, this is the first time I've seen this in a while.

It might be worth getting in touch, since I wouldn't be very inclined to log 
in, at least through
http://www.coreconceptsclarence.com
having to allow cookies from everywhere.
